New Jersey Physicians ACO Selects eClinicalWorks for Population Health

WESTBOROUGH, Mass.—September 15, 2014—eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced that New Jersey Physicians ACO has selected eClinicalWorks Care Coordination Medical Record® (CCMR) for population health management to advance its Accountable Care Organization (ACO)-related objectives. This relationship will aid the physician-led organization in administering the ACO in a more efficient manner, including capturing quality metrics and performing analytics.

The ACO care delivery model coordinates care between patients and members of the healthcare team with the goal of helping strengthen the physician-patient relationship. Recently recognized by Frost & Sullivan with its 2014 North America Frost & Sullivan Award for Enabling Technology Leadership, eClinicalWorks CCMR will work closely with New Jersey Physicians ACO in meeting its quality measures and performance metrics.

Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
