New Study: EHR Use Increases Physicians Total Revenue

The CMS and HHS' Office of the National Coordinator issued their much-anticipated final meaningful-use information technology regulations that hospitals and physicians must follow to tap into some $27.3 billion in financial incentives authorized by the stimulus act.

Providers appeared most interested in learning whether the CMS would modify a proposed rule mandating an all-or-nothing, “pass-fail” approach dictating that, in order to receive a subsidy, doctors must meet, in its entirety, a list of 25 requirements and hospitals must meet a similar list of 23, and the CMS appeared to have been receptive.

House Passes Bill To Create EHR Loan Program for Physicians

Yesterday, the House of Representatives approved the Small Business Health IT Financing Act and sent the measure to the Senate. The bill would create a Small Business Administration loan program to help physicians purchase electronic health record systems. Last week, Sen. John Kerry introduced a similar bill in the Senate. EHR Procrastinators could face series of tighter standards

There may soon be one more incentive for hospitals and physician offices to buy and install electronic health-record systems on or before 2011. The added push could come from the prospect of increasingly higher thresholds of initial federal eligibility requirements for EHR subsidies under the American Recovery and Reinvestment Act of 2009, according to discussions at Tuesday's meeting of the Health Information Technology Policy Committee.
A work group of that committee delivered its first draft of recommended definitions of " meaningful use" of EHRs, a standard that providers must meet to qualify for subsidy payments estimated at $34 billion to be handed out by Medicare and Medicaid. The work group recommended instituting a series of increasingly complex meaningful-use requirements between 2011, the first "payment year" of the subsidy program, and 2015, the final year payments will be made before financial penalties for not adopting begin.
During those discussions, Anthony Trenkle, director of the CMS' office of e-Health Standards and Services, said the requirements will not be "tiered" based on when the provider adopts an EHR after 2011. Instead, whatever meaningful use standards are applicable for the year the provider applies for an EHR subsidy are the standards that provider must meet, regardless of whether it is the provider's first year of EHR implementation.
A 10-day public comment period is now open on the work group's initial recommendations. Trenkle said the CMS hopes to have a final definition of "meaningful use" to put out for a 60-day comment period later this year, with final rulemaking not expected until early next year.

To qualify for the $44,000 in Stimulus payments under the Medicare plan, medical providers must demonstrate meaningful use of a certified EHR system starting in 2011. Implementing and demonstrating meaningful use of the EHR could take some time and thus should be started soon by most providers. This may present a financial timing problem for some providers as the need to fund the implementation of the EHR occurs prior to the receipt of the payment s under the HITECH plan.

Implementing the new ICD-10 codes will cost the typical small medical practice (three doctors and two administrative staffers) $83,200; the typical medium-size practice (10 providers, six staffers, one full-time coder) $285,195; and the typical large practice (100 providers, 54 medical records staffers, 10-full-time coders) almost $2.73 million, according to a new study conducted by Reisterstown, Md.-based Nachimson Advisors, a healthcare information technology consultant. The study was funded by 11 medical organizations, including the American Medical Association and the Medical Group Management Association, and it researched the costs associated with replacing the current ICD-9 codes with the new, more detailed ICD-10 system by 2011 as proposed by HHS.
